Do you issue Medical Certificates?

Yes. Take note that a medical certificate is a legal document affixed with my signature so there will be an additional charge of 250 php. Medical certificates shall only be given after the consultation. These cannot also be used for medicolegal purposes.

Can I message you via the NowServing app?

Yes, you can send me a message through the NowServing app once I already accepted your appointment. You will receive a reply as soon as possible.

Do you issue "Fit-to-Work" Clearances?

Yes. Please do note that it is a legal document bearing my signature and medical license hence an additional 250 php charge will be required. Clearances may not be used for medico-legal purposes.

Can I book a schedule today?

Yes. As long as there is an available schedule, requests for appointments will be immediately accommodated. If not accepted within 24 hours, your request will be automatically cancelled by the system.

Do you issue CP (Cardiopulmonary) clearances before my operation?

Yes. Please do note that depending on the nature of the procedure and the patient's condition, an additional charge for the clearance will be required.

Can I book a face to face consultation with you?

Yes. I have a physical clinic in San Jose District Hospital in San Jose, Batangas on Tuesdays and Fridays 9AM-4PM. Walk-ins only accepted.
