Informed Consent
Patient records are securely stored and accessed by Roland Abina-Japone’s Mental Health Team. Confidentiality exceptions include A. serious harm (to self or others), B. abuse, C. legal orders, or D. patient consent.
Therapy success depends on both the therapist and the patient. The patient is responsible for lifestyle changes from therapy. The therapist isn’t obligated to appear in legal proceedings unless legally required.
If the therapist perceives danger to the client, they can warn the person in danger or contact authorities. This expires upon service termination.
The client can terminate services anytime.
Consultation notes will be taken for progress monitoring. Data will be securely stored and discarded after service termination or up to 15 years as per the Department of Health’s recommendation.

Do you issue Medical Certificates?
No. But alternatively, I issue a Mental Health Certificate that serves the same purpose. It's our (Psychologist) own version of a clinical certificate. Note that this form of clinical certificate is a legal document affixed with my digital signature. Clinical certificates that indicate leave recommendation are not part of insurance coverage and shall only be given for specific valid reasons and after the consultation.
