Informed Consent for Teleconsultation
By booking a teleconsultation, you acknowledge and agree to the following:
Confidentiality: Your records are securely stored. Information will only be disclosed under these circumstances:
1. There is a serious risk of harm to yourself or others.
2. Abuse or neglect is disclosed or suspected.
3. A legal order requires disclosure.
4. You provide written consent.
Therapy Process: Successful therapy requires active participation from both the therapist and the client. Clients are responsible for applying strategies discussed during sessions. The therapist is not obligated to participate in legal proceedings unless legally mandated.
Risk Management: If the therapist identifies a risk of harm, they may notify the person at risk or contact appropriate authorities. This authorization ends when services are terminated.
Service Termination: You may discontinue services at any time. Consultation notes are securely stored and retained for up to 15 years, in compliance with DOH guidelines.
Session Duration
A typical session lasts approximately 45 minutes per client.
The first session is dedicated to intake interviews and initial assessment.
Duration may vary depending on individual needs.
