Do you issue Medical Certificates?

Yes. Take note that a medical certificate is a legal document affixed with my signature, so there will be an additional charge of 400 pesos. Medical certificates are not part of insurance coverage and shall only be given for specific valid reasons and after the consultation. Issuance of a medical certificate is subject to my professional discretion, based on the findings from the consultation. I reserve the right to refuse issuance if there is no valid medical basis, in accordance with ethical and legal guidelines governing medical practice in the Philippines. Any attempt to misrepresent symptoms or request a certificate for fraudulent purposes may be grounds for refusal and may be reported if necessary.

Do you issue S2 prescription?

Yes. S2 prescriptions are required for restricted drugs, such as stimulants and benzodiazepines. I charge an additional fee of 400 pesos to cover logistics, time, and the cost of maintaining the S2 license, which is a special requirement for issuing these prescriptions.
