Yes. If you indicated PWD, please upload a valid ID before your schedule; there is an additional fee for invalid PWD IDs. Requests for cancellation or reschedule may be accepted at least 12 hours before the schedule. The professional fee is refundable or can be carried over only for cancellations or reschedules made, respectively, at least 12 hours before the schedule. Otherwise, there will be an INCONVENIENCE FEE for NO-SHOWS (i.e., no prior notice 12 hours from your schedule) before we can refund the professional fee.

Can I be late? Can I be late in sending labs and photos?
Please take note that we will need to cancel your appointment IF WE DO NOT RECEIVE A RESPONSE FROM YOU WITHIN 15 MINUTES after your schedule to give way to the other patients on queue. This will be tagged as NO-SHOW, which has a corresponding INCONVENIENCE FEE.
If you have any pertinent RESULTS TO SHOW, please scan/ screenshot them clearly and send to this platform BEFORE your schedule in order to save time; thank you
Do you accept international patients?
Yes.
HMOs are accepted only for clinic visits, which you can arrange thru SMS to 09955782885. We do not accept HMOs for teleconsult. If you do not have a teleconsult schedule yet, please make a booking thru https://seriousmd.com/doc/cchavez. All instructions are written on the booking page. Please read thoroughly and be guided accordingly.
Can I book a schedule today?
Please refer to the doctor schedule indicated. FIRST TO BOOK, FIRST TO BE SCHEDULED BASIS. The slot you will be accepted to is the soonest available. For urgent concerns, please go to any available emergency room.
If I cancel my request for consultation, do I get a refund?

Which HMO's/Healthcards do you accept?

Do you issue Medical Certificates?
Yes. Take note that a medical certificate is a legal document affixed with my signature and shall only be given for specific valid reasons and after the consultation. Medical certificates are not part of insurance coverage. For HMO (applicable for clinic visits only), there will be an additional charge of Php 200.
