Do take note that the consultation is for nonemergency cases only, if you have concrete and immediate plans of suicide or harm to others kindly go to the nearest emergency room. Thank you

If a patient is a minor (Below 18 years old), should there be a written consent from parents/legal guardian?

Yes. A picture of written consent for consultation from a parent/legal guardian must be sent to the doctor upon acceptance of consult. The parent/legal guardian will be informed of the diagnosis and psychopharmacologic treatment but all OTHER ISSUES NOT CONCERNING HARM TO SELF/OTHERS will still be held CONFIDENTIAL between patient and therapist

Do you issue Medical Certificates?

Yes. Take note that a medical certificate is a legal document affixed with my signature so there will be an additional charge of 300 pesos. Medical certificates are not part of insurance coverage and shall only be given for specific valid reasons and after the consultation.
