I do not accept any HMO's or Health cards at the moment.

Do you issue Medical Certificates?

YES. Take note that a medical certificate is a legal document affixed with my signature so there will be an additional charge of Php 150.00. Medical certificates are NOT part of insurance coverage and shall only be given for specific valid reasons and after the consultation. Please be informed that it will be EXPLICITLY stated in the medical certificate that the check up was done via teleconsultation, hence there will be limitations in fully examining the patient. Furthermore, I will ONLY write the date of the ACTUAL consultation on the medical certificate. Antedating (i.e. backdating) a legal document will NOT BE DONE.
