Do you issue a Mental Health/Psychological Certificate?

Yes. Take note that a psychological/mental health certificate is a legal document affixed with my signature so there will be an additional charge depending on the purpose. Mental Health/Psychological certificates are not part of insurance coverage and shall only be given for specific valid reasons and after the consultation. Please do note that the psychological/mental health certificate may need more than 1 session depending on the case. Alternatively, you may request a certificate of consultation indicating that we had a session and if needed, the schedule of succeeding sessions. (additional fees may apply). Psychological Assessment may be necessary depending on the need and reason for referral (additional fees may apply).

Is there an Informed Consent before the session?

Yes. The informed consent and intake forms will be sent to you once you have an agreed upon schedule with your psychologist. Please accomplish it at the latest one hour before the session to avoid cancellation/rescheduling of the session.

Cancellation Policy

Cancellations may be made at the latest 3 hours before the session. If cancellation is done after, only 50% refund may be given. This is to respect the time and preparation for the psychologist.

No Show Policy

No Shows will be tagged once the client did not respond to messages sent or did not go to the agreed schedule for 15 minutes. No show will lead to forfeiture of your payment as to respect the psychologist's time.
