I want to use my HMO card for free teleconsults. How do I get a LOA?

For Maxicare card holders, please secure a LOA through https://membergateway.maxicare.com.ph/ Click Request LOA > Consultation > Doctor's name: COSTINIANO and fill in other required details. Alternatively, you may also call Maxicare 24/7 Customer Care Hotline: (632) 85821900 /77987777 to secure your LOA. For Intellicare/AVEGA card holders, you may get an e-LOA by downloading the AGORA app and requesting LOA for teleconsult. For Medicard users, please download the MACE app and request a LOA. Once with LOA, please book an appointment through this website and input CORRECT MACE ID. Please take note of your OTP. For PhilCare card holders, you may access https://philcare.com.ph/gateway/login or download the HeyPhil Digimed app and generate the LOA.

Alternatively, you may also call Maxicare 24/7 Customer Care Hotline: (632) 85821900 /77987777 to secure your LOA. Once with LOA, please book an online appointment at https://seriousmd.com/doc/jbrian and choose your preferred appointment date. You may message your preferred time of consult as well. Online clinic is everyday from 8am-10pm.

Do you issue Medical Certificates?

We can issue pre-employment medical evaluations, fit to work/travel certificates and other relevant documents except medicolegal cases upon full consult and evaluation depending on the discretion of the physician.
