Do you issue Medical Certificates?

No, but I can provide you a certification of completed Diet and Nutrition Consultation.. Take note that this certificate is a legal document affixed with my signature, so there will be an additional charge of Php 100.00. This Certificate shall only be given for specific valid reasons and after a consultation.
