Do you issue Fit to Work/Cardio Clearance for Employment?

Yes! It will depend on your clinical status and the type of work you are applying for. Take note that this medical certificate is a legal document affixed with my signature so there will be an additional charge of [Php 500.00]. Medical certificates are not part of insurance coverage and shall only be given for specific valid reasons and after the consultation.

Do you issue Risk Stratification (RS) /CP clearance for a procedure?

Yes. Depending on the procedure and your clinical status, the basic/minimum diagnostic procedures needed are: 12-L ECG, Chest X-ray, CBC, FBS/Hba1c & creatinine. In some cases we might need: PT/PTT (bleeding parameters), 2D echo, electrolytes. RS/CP clearance will be issued in a medical certificate issued after the consultation and is a legal document affixed with my signature so there will be an additional charge of [Php 1,000.00 for senior citizens; Php 1,500.00 for non-seniors].
