Do you issue Medical Certificates?
Yes. However, please note that I can only issue medical certificates for patients I have seen/examined on the date of consult. I cannot backdate a med cert, in compliance with the law and ethical standards as a professional. A medical certificate is a legal document affixed with my signature so there will be an additional charge of Php 50. It is not part of insurance coverage and shall only be given for specific valid reasons and after the consultation. This can also be easily validated by your company through my provided contact details on the medical certificate.
How does my company validate a medical certificate?
Your company can contact me through my provided phone number on the medical certificate. As much as possible, I am able to reply to them within minutes for the validation.
What are your accepted payment methods?
I can accept payment through GCash, PayMaya, and online banking (accounts are: BDO, CIMB, MariBank). The details will be given during the consultation.
