Do you issue Medical Certificates?

Yes. Take note that a medical certificate is a legal document affixed with my signature so there will be an additional charge of Php 300. Medical certificates are not part of insurance coverage and shall only be given for specific valid reasons and after the consultation.

Which HMO's/Healthcards do you accept?

HMO's (i.e. Maxicare, Medicard) are accepted only for face-to-face consultation at Fatima University Medical Center Antipolo. If you have another HMO not listed above but interested in scheduling there, kindly send us a message through our facebook page to ask and we will check if your HMO can be accepted. For online consultation through SeriousMD, HMOs are currently not accepted.
