Do you issue Medical Certificates?

Yes. Take note that a medical certificate is a legal document affixed with my signature so there will be an additional charge of Php100. Medical certificates are not part of insurance coverage and shall only be given for specific valid reasons and after the consultation.

What do I need to prepare before my consultation?

For a smoother consultation process, please send the following documents (if applicable) before your appointment: > A copy of your laboratory results > A copy of any pertinent medical files related to this consultation > A copy of LOA that has been signed by the patient
